Formal Registration By Staff/Professional

This form is for staff and professionals to formally register a child with the New England Consortium on Deafblindness (NEC). To complete this form, personnel must submit child evaluations (vision and hearing reports) and releases signed by parents. Staff and professionals submitting a formal referral will receive notices of NEC resources and events.


Formal Registration By Family

This form is for parent/guardian looking to register a child with the New England Consortium on Deafblindness (NEC). To complete this form, parents must submit child evaluations (vision and hearing reports) and signed parent releases. Parents submitting a formal registration will receive notices of NEC resources and events.


Informal Referral By Staff/Professional

This form is for staff and professionals looking to submit an informal referral when (a) the child is "at risk" for combined vision and hearing loss, (b) the family is not ready at this time, and/or (c) evaluations are incomplete. The informal referral gathers limited information only. The family and child cannot be identified in any manner, but can be counted in the state deafblind census. Staff and professionals submitting an informal referral will receive notices of NEC resources and events.
