NEC’s Transition Team Initiative (NEC-ITTI)
This training project incorporates evidence-based practices for teens and young adults who have combined vision and hearing loss, their families and educational team members. These practices promote future opportunities for greater independence and improved quality of life.
Transition planning for young adults who have combined vision and hearing loss requires an organized team approach, involving the student, family, and team members. NEC’s Transition Team Initiative will bring together families, professionals and agencies in order to complete the following:
- Develop a person-centered plan and educational program
- Incorporate family involvement throughout program development and future planning
- Identify key supports and services provided by state and local agencies
- Increase service provider skills in assessment, program development and implementation as outlined in the Transition Action Plan.
Step 1: Meet with IEP/Transition Planning team; identify important team members and agencies; and develop Person Centered Plan
Develop Transition Plan and identify critical steps toward success
Step 2: NEC personnel will facilitate monthly sessions (virtual and/or face-to-face), using guided tools and short webcast presentations
Step 3: Final face-to-face session
